Your initial appointment will consist of a comprehensive evaluation and consultation explaining your diagnosis and treatment options. A comprehensive evaluation starts with a thorough examination of the teeth and gums, x-rays, models, photos and an evaluation of your overall health.
Sometimes the solutions seem very simple and can be discussed at that time. However, for complex cases, we suggest a separate consultation appointment to discuss your treatment options with Dr. Sugar and your budgeting options with the treatment coordinator.
Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
For your convenience, new patient forms are available to complete, print, and bring with you at your first appointment. This will expedite your registration process.
Our purpose is to help you understand your dental needs, identify your desires, and exceed your expectations.
Unless there is an urgent need for cosmetic, restorative or maintenance care, your first appointment will emphasize getting to know you and starting the process of creating a personalized treatment plan for caring for your unique needs. If you have a specific goal in mind we want to make it happen for you!
Please assist us by providing the following information at the time of your consultation:
1. List of your current medications
2. Consider what you would like to have for the health and appearance of your smile in 5 or 10 years.
3. If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under 18 must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
